Bug report form template, what happened and what you expected.

featurevote has a free bug report form template. People describe what happened, attach a screen recording, video or screenshot, and leave an email to hear when it is fixed.

With Known issues on, bugs get their own tab on your board. People see the ones you already know about and say me too instead of reporting them again.

Turn on Known issues in your board settings to collect bug reports.

— The form

What is in the template?

This is the form on a featurevote board with Known issues on, in the order people see it.

Report a bug
People pick a bug or an idea when they post, so bugs stay off your roadmap.
What happened, and what did you expect to happen?
The one required box. Add screenshots, upload a video, or record your screen with optional microphone audio. Preview a recording before attaching it.
Title (optional)
Left blank, the board writes a short title from the report.
Already known?
Before a new bug goes up, people see up to three known bugs that match and can say me too instead.
Email
How they hear when it is fixed. It is never shown publicly, and the Visitor email setting can make it optional.
Name (optional)
Shown on the report if they give one.

— After someone sends it

Where do the reports go?

A form tool saves each report as a row that only your team sees, so the same bug can come in many times.

On a featurevote board each bug shows how many people said me too, and whether it is reported, confirmed or being fixed. Your note can sit on the bug, so people find a workaround without asking. A fixed bug moves to Completed, and you can email the people who reported it or said me too, if they left an email.

The known issues page shows the tab in more detail, and the feature request form template shows the form for ideas.

— Sharing it

How do people get to the form?

Turn on Known issues in your board settings, on any plan. The Known issues tab has its own link, so you can share it in your docs, your Discord or a support reply, and people report bugs from there.

The feedback widget on your site posts ideas, not bugs, so for bugs link people to the tab.

— Pricing

What does it cost?

The free plan is permanent. People suggest and vote without limit, and your team can manage as many ideas and bugs as it likes. Paid plans add a bigger email list and start at $9 a month, billed yearly. See pricing.

When your QA team needs fields like device, version and steps to reproduce, an issue tracker or a longer form is the better fit.
